All Patients are encouraged to contact their insurance plan to find out if they have a copay, coinsurance, or have met their deductible, prior to their appointment. This is information I will need when we meet.
Important:
If you have an HMO or a Medicaid plan that requires a referral from your primary care physician, I must have an authorization number faxed or phoned to my office prior to our appointment. If I do not have it two days in advance, I will have to call and reschedule your appointment. A referral alone is insufficient. If you are not sure, please call your doctor's office to make sure the authorization has been faxed to me at 602-262-2223.
